Czech health insurance
As with most subjects linked with visas etc. the system is split into EU and non EU.

Czech health insurance for third country nationals
Due to the amount of fake health insurance cards coming from outside the Czech Republic, in 2010 the government introduced controls on health insurance companies and health insurance when related to visas and residency permits.In particular health insurance linked to visas longer than 90 days and residence permits.
If you plan on staying in the Czech Republic for more than 90 days then you will need health insurance from a Czech health insurer or health insurer that is accredited with the Czech National Bank.

If your (non Czech) health insurance company is registered with the CNB then it is likely that your insurance is valid for your visa. Although this is not always the case.
Health insurance for a Czech visa/residence permit
When applying for your long term visa you do not need Czech health insurance. But, when you collect your visa you will be required to prove that you have sufficient health insurance to cover you for the length of your visa.The Czech heath system
