• a travel document;
• 1 photograph;
• a completed form “Application for Schengen Visa”;
• a document on availability of funds to cover the stay in the territory of the Contracting Parties and a document on availability of accommodation for the entire period of permitted stay, namely:

- an invitation verified by the local alien police inspectorate.

• an invitation letter of the business/firm, written on a letterhead, which specifies its holder's identification data, i.e. at least the holder's name, surname, date of birth, nationality, period of stay (from – to), information about payment for services provided in the territory of the Czech Republic in lieu of funds to cover the stay, about the provision of accommodation and which includes the reason of the invitation. the stamp of the issuing legal entity, signature of the issuing legal entity's employee, the date of issue (unless you present an invitation verified by the local alien police authority);
• a document certifying the availability of funds for the stay in the territory of the Czech Republic, listed in Section 13 of Act No. 326/1999 Coll., or an invitation for a visit to the Czech Republic verified by an alien police inspectorate (unless you present one of the documents described in the previous paragraphs);
• a document on provision of accommodation during the entire stay in the territory of the Czech Republic (unless you present one of the documents described in the previous paragraphs);
• a proof of availability of funds to cover the costs related to departure (unless you present one of the documents described in the previous paragraphs);
• travel medical insurance for an amount of at least 30,000 EUR, covering the entire Schengen Area (such insurance shall be presented before affixed the visa in the passport; however, we recommend presenting it at the time of submission of the application);
• in case of application for a single-entry visa, the insurance has to cover the entire length of stay, including the transport period;
• in case of application for a dual or multiple-entry visa, the insurance has to cover the first journey or the entire length of stay + a solemn declaration;
• a proof of payment of the fee for submission of the visa application

You may be requested to submit supporting documents, such as:

- a medical report certifying that you do not suffer from a serious disease;

- further documents certifying the data stated in the visa application, e.g. a copy of a return air ticket.

Note

!!! Supporting documents accompanying the application can be mutually combined; therefore, it is up to you to select the way to document the availability of funds to cover your stay in the territory of the Czech Republic.
